You must file a tax return, plus timely filed extensions, by the due date to avoid a failure-to-file penalty. Having a reasonable cause for missing the due date may help avoid the penalty. Again, having a reasonable cause for failing to pay may be avoided if you have a reasonable cause for your failure to pay.
