VICTIM OF IDENTITY THEFT?

If you're the victim of identity theft, do the following:
- Check your bank accounts and report any suspicious activity to your bank
- Report the crime to your local police
- Report the crime to the Federal Trade Commission (FTC) at IdentityTheft.gov
- Request a freeze and a fraud alert on your credit from the national credit
bureaus: Equifax
(800-525-6285), Experian (888-397-3742), and
TransUnion (800-680-7289)
- Respond immediately to any IRS notices received in the mail by calling the phone number provided
- Request a copy of any fraudulent tax return from the IRS
- File with the IRS an Identity Theft Affidavit Form
14039, Identity Theft Affidavit
- Close all accounts opened in your name by the identity thief
